What is a GP chronic condition management plan?
A GP chronic condition management plan is a plan you make with your GP to coordinate care for a long-term condition such as type 2 diabetes. It sets goals and arranges regular reviews.
Regular diabetes checks
- HbA1c blood tests to track blood sugar over time
- Blood pressure and cholesterol
- Kidney function tests
- Foot checks
- Regular eye checks
Allied health support
Eligible patients may be able to access Medicare rebates for allied health services such as dietitians, diabetes educators, podiatrists and exercise physiologists.
Lifestyle matters
Healthy eating, regular physical activity, not smoking and good sleep all support diabetes management. Your GP can help you set realistic goals.
Costs
GP chronic condition management plans and reviews are bulk billed for eligible patients with a valid Medicare card.
