Chronic condition management plan

Regular reviews

Since 1 July 2025, GP chronic condition management plans have replaced GP Management Plans and Team Care Arrangements. Your GP prepares a plan with you and reviews it regularly to keep your goals and care on track.

What it involves

  • Sets goals for conditions such as diabetes, heart disease, asthma or arthritis
  • Coordinates care with allied health providers
  • Reviews keep the plan current

Good to know

  • To access Medicare rebates for allied health services, your plan generally needs to have been prepared or reviewed within the previous 18 months
  • If you are registered with MyMedicare, your plan should be with your registered practice

Billing: GP consultations are bulk billed for eligible patients with a valid Medicare card.

Mental health care plan

Review after initial sessions

A mental health care plan can give access to Medicare rebates for sessions with a psychologist or other mental health professional. Your GP reviews how you are going and whether further sessions would help.

What it involves

  • Up to 10 individual sessions per calendar year under Better Access, with a GP review along the way
  • Referrals to psychologists, social workers and occupational therapists

Good to know

  • Book a longer appointment for a plan or review
  • Crisis support any time: Lifeline 13 11 14

Billing: GP consultations are bulk billed for eligible patients with a valid Medicare card.

Health assessments

Yearly or as eligible

Structured health assessments, such as for people aged 75 and over, or Aboriginal and Torres Strait Islander people, look at your overall health and plan ahead.

What it involves

  • Longer appointment
  • Our nurse may help with part of it

Billing: GP consultations are bulk billed for eligible patients with a valid Medicare card.

What happens at a care plan review.

Allied health services

With an eligible chronic condition management plan, Medicare rebates are available for up to five allied health services each calendar year, such as physiotherapy, dietetics, podiatry, diabetes education or exercise physiology. Allied health providers set their own fees.

  1. Tell reception it is a care plan review, so enough time is allowed.

  2. Your GP looks at how you are going against the goals in your plan, and what has changed.

  3. Your GP reviews recent results and may order tests, such as blood tests, before or at the review.

  4. Goals, treatments and allied health referrals are updated, and the next review is planned.
